📌 What is TRD?
TRD is defined as major depressive disorder that does not respond adequately to at least two different antidepressant treatments used at correct dose and duration.
Such patients continue to experience significant symptoms affecting their daily life.

USFDA-approved treatments for TRD:
-Esketamine (Spravato nasal spray):
-First medication approved specifically for TRD
-Rapid onset (can work within hours)

Given under medical supervision along with oral antidepressant

Olanzapine + Fluoxetine (Symbyax):

Approved for TRD

Combines an antipsychotic and an SSRI,
Role of Ketamine in TRD:
Works differently (NMDA receptor antagonist → affects glutamate pathway)

Advantages:
✅ Rapid relief, even in suicidal patients
✅ Helps patients who failed other medicines

Side effects:

Dissociation, dizziness

Raised blood pressure, nausea

Rare risk of misuse if not supervised

Must always be given under trained medical care (IV infusion or nasal spray).


Other important options & their status:
Aripiprazole & Quetiapine:

USFDA-approved as augmentation (add-on) in major depression

Widely used in partial/non-responders, though not specifically approved for TRD

rTMS (Repetitive Transcranial Magnetic Stimulation):

FDA-approved for depression

Non-invasive option, especially when medicines fail

ECT (Electroconvulsive Therapy):
Who Is at Risk for Treatment-Resistant Depression?
 You may be more likely to develop it because of a few things. Risk factors for this condition include if:
• The onset of your depression began at an earlier age
• You have more frequent and recurring depression episodes
• Your depression episodes last longer
• You have a severe case of depression
• You’re older in age
