Emergency Department (ED) overcrowding has a direct impact on timely behavioral health admissions, with Maryland’s average wait times exceeding the national median by 1.5 hours. As the state’s largest nonprofit behavioral health provider, Sheppard Pratt operating more than 400 licensed beds across two hospitals recognized the urgent need to streamline intake processes to reduce delays and improve access to care.

The Operational Excellence team used Value Stream Mapping (VSM) to identify inefficiencies and implement 13 targeted initiatives. These included redesigning the patient waitlist through EMR integration, centralizing referral decisions with a new “Screening Provider” role, and creating a stoplight communication system for admission capacity. Additional steps standardized discharge notifications, added daily management huddles with dashboards, introduced discharge forecasting, launched an ambulette service, and created the Admissions Shift Coordinator role with enhanced staff training.

Results were measurable: discharge notifications reached the bed board within 30 minutes, compliance with noon discharge targets increased, and ED-to-bed transfers accelerated, leading to higher occupancy rates and reduced patient wait times. By aligning technology, staff roles, and communication systems, this initiative improved patient flow and overall care coordination. The project demonstrates how structured process improvement and interdisciplinary collaboration can enhance operational efficiency and patient experience in behavioral health systems.
