Intended Audience
This webinar is intended for nurses who care for people with heart conditions, stroke or vascular cognitive impairment across all settings and stages of care (e.g. acute care, community), however all healthcare providers are invited. This is an opportunity to have discussions and hold collaboration with Heart & Stroke, the National Emergency Nurses Association, the Canadian Association of Neuroscience Nurses, and the Canadian Council of Cardiovascular Nurses

About this presentation
The Heart and Stroke Foundation provides guidance for healthcare providers supporting people who have experienced heart conditions, stroke and vascular cognitive impairment. The 2019 Heart and Stroke Report examines the connections between these conditions. Nurses have a unique first-hand experience of these strong associations through the care they provide. In recognition of Nursing Week, Dr. Patrice Lindsay and Dr. Cindy Yip will discuss the research presented in the 2019 Heart and Stroke Report and how this relates to direct nursing care.

Learning Objectives
Participants of this program will be able to:
Explain the connection between heart conditions, stroke and vascular cognitive impairment.
1. Identify key challenges experienced by people who have experienced these conditions.
2. Engage in meaningful conversations with people, their families and caregivers about their experiences navigating the health care system and opportunities for improvement.
3. Advocate for seamless transitions of care for people with multimorbidities and the role nurses play.
