Damage to the Skull in a Car Accident:
Damage occurs because the person receives a blow to the head that whips the head forward and back or from side to side (as in a car crash), causing the brain to collide at high velocity with the bony skull in which it is housed. This jarring bruises brain tissue and tears blood vessels, particularly where the inside surface of the skull is rough and uneven; damage occurs at (and sometimes opposite) the point of impact. This disruption in the cellular function or structure is classified as a closed head injury

Types of Closed Head Injuries (as determined by an MRI):
1. Concussion
2. Contusion
3. Diffuse Axonal
4. Coup-Contrecoup

Types of Diagnosis:
History - mechanism of injury or impact (speed, restrained driver), details of trauma (loss of consciousness, additional blunt injuries)

Examination - neurologic examination including Glasgow Coma Scale (GCS) (eye opening, verbal response, motor response)

Imaging - CT head, MRI brain

Treatments:
MILD TBI -
Rest
Abstain from physical activity
Return to normal routine following repeat examination by provider and ongoing support of rehab team

SEVERE TBI -
Treat/manage structural injury (stop bleed, drain bleed, relieve pressure, monitor pressure, limit inflammation)
Monitor for progression of symptoms with supportive treatment and sub-specialist consultation
Modify lifestyle for persistent deficit

Prognosis:
mild TBI - days to weeks to recover
severe TBI - months to recover (if structural injury some neurologic function may be permanent)

Lasting Effects:
depends on pre-incident functional and cognitive levels
mild TBI - minimal lasting effect
severe TBI - fluctuation in or persistent neurologic/sensory/motor dysfunction
