Disseminated Intravascular Coagulation (DIC) is a life-threatening condition characterized by widespread activation of the coagulation cascade, leading to the simultaneous formation of microthrombi in small blood vessels and excessive bleeding due to the depletion of clotting factors and platelets. This imbalance occurs secondary to severe conditions like sepsis, trauma, obstetric complications (e.g., abruptio placentae, amniotic fluid embolism), malignancies (e.g., acute promyelocytic leukemia), or severe burns. The formation of microthrombi causes ischemia and organ dysfunction, while the depletion of coagulation factors results in uncontrolled bleeding. Clinically, DIC presents with bleeding from multiple sites (gums, GI tract, surgical wounds), petechiae, ecchymoses, and symptoms of organ failure like dyspnea, oliguria, or altered mental status. Diagnosis relies on laboratory findings, including prolonged PT and aPTT, elevated D-dimer levels, and decreased fibrinogen and platelet counts. Management focuses on treating the underlying cause, supporting coagulation with fresh frozen plasma (FFP) or platelet transfusions, and, in selected cases, using heparin to control thrombosis.
