Beginning in January 2025, all primary care practices applying or re-applying for PCPCH recognition will do so under the 2025 PCPCH Recognition Criteria and Standards (see link below). A webinar series was created to assist practices in understanding some of the changes under the 2025 iteration of the model.

This session (July 12, 2024) provided an overview of the updated version of PCPCH Standard 3.E - Preventive Service Reminders, which includes revisions to point values, the addition of a new measure centered on developing an alternative reminder or outreach strategy for patient subgroups experiencing disparities in utilization of preventive services, and updates to the expectations around tracking of preventive services to align with typical processes.
