What key insights will you be sharing in your session and why is this important?

Dr. Silvia Poli: Have you ever asked people living with progressive MS what really matters to them, and then work with them to make that happen? That's exactly what our Living Well Program aims to do.

My name is Silvia Poli, and at ECTRIMS RIMS 2025, I will be presenting together with Prof. Das Nair, our research funded by the International Progressive MS Alliance on supporting people with progressive MS, identifying and working towards goals that are truly meaningful to them.

Very often support is framed around symptom management. While important, this doesn't always reflect what individuals actually want or need in their daily lives. Moreover, programs usually are focused on a single symptom or area, while engagement in meaningful activities can be limited by multiple and diverse problems.

Our well-being program shifts the focus by placing the person, not the condition, at the center. The Living Well Program is a holistic, personalised, multi-symptom, and goal-oriented approach grounded in shared decision-making, person-centeredness, and coaching philosophies. With each person, we identify the barriers that limit engagement invalid activities and explore their individual strengths and resources to co-create a tailored, meaningful plan.

The program uses a case coordinator model where trained coordinators work closely with individuals and are supported by a multidisciplinary team, including psychologists, nurses, occupational therapists, and physiotherapists, ensuring that every solution is grounded by a psychosocial perspective.

We have piloted the program across three countries, and I will be sharing insights from our experience, the challenges we faced, and the opportunities this model offers for personalised, meaningful care. I'm truly excited to be part of ECTRIMS RIMS 2025 and to exchange ideas on how we can reimagine support for people with progressive MS.

Why are you most looking forward to at ECTRIMS 2025?

Dr. Silvia Poli: I'm looking forward to have the chance to connect with people, having different perspectives, coming from different disciplines, all working together towards the same challenges and the same objective. I'm also excited to hear new thought-provoking research that challenges how we think and how we work. And I'm inspired by the opportunity to be part of something that truly matters — improving the lives of people with MS in ways, that are meaningful to them. And doing all this in the wonderful city of Barcelona... what's not to look forward to?
