Many people are working beyond their Medicare eligibility at age 65 and opting out of Medicare while working because they have employer medical insurance from their job or possibly a spouses current employer. But what happens when that employer insurance ends and you need to enroll into Medicare?

This video will go step by step how to complete the CMS-40B application for Medicare & CMS-L564 Employer attestation form. Both forms are required when you wish to enroll into Medicare after employer group insurance coverage has ended and you are at least 65 and 4 months old.

When these 2 forms are completed and properly submitted to Social Security administration, you can succesfully enroll into Medicare and avoid late enrollment penalties or possibly delayed effective dates of coverage.
