Scoliosis bracing has long been one of the most common non-surgical treatments for scoliosis. At one time, it was considered the primary approach before surgery became more common. The invention of the pedicle screw shifted treatment, but bracing continues to play a crucial role.

Various types of braces have been designed over time to wrap around the torso, primarily aiming to prevent scoliosis progression. Traditional braces focused on stopping worsening, particularly during growth spurts. It wasn’t until modern corrective bracing emerged that treatment began focusing on not just halting progression but also improving spinal alignment.

Bracing is most effective on flexible spines, which are typically found in growing children. However, bracing can still be beneficial for adults, as there are ways to improve spinal flexibility even after growth has stopped. Traditional braces are mostly used during periods of rapid growth to prevent significant progression.

When it comes to sleeping in a brace, it depends on the type of scoliosis and the prescription. Every case is unique, and brace-wearing guidelines vary. A brace that isn’t worn offers no benefit. Some patients are prescribed braces for part-time use, while others need full-time wear. Patients who wear a brace part-time often choose to sleep in it to fulfill wear-time requirements. For full-time prescriptions, sleeping in the brace is necessary.

Certain braces are designed specifically for nighttime use. These braces cannot be worn while standing, walking, or performing daily activities. Known as nighttime braces, they hold the spine in an over-corrected, bent position during sleep. A common example is the Boston Bending Brace, often used for lower spinal curves like lumbar or thoracolumbar scoliosis. These braces are less effective for higher curves, where it’s harder to bend the spine.

Sleeping in a brace might feel uncomfortable at first, but it typically becomes easier over time. If the brace starts feeling more comfortable, it’s likely a sign that the spine is improving. Conversely, if discomfort increases, it could signal that the curve is worsening or that the brace is no longer the right fit.

Nighttime bending braces come with limitations. They are often difficult to wear due to the fixed position they hold the body in. The effectiveness of these braces is also limited by how many hours the patient sleeps. If the patient sleeps for six to eight hours, the ""dose"" of treatment is low.

Nighttime bending braces, like the Providence or Wilmington braces, tend to address only one plane of scoliosis—the lateral bend—while neglecting rotational and vertical components.

Traditional braces, like the Boston brace, also have limitations. They squeeze the spine to hold it in place, without fully addressing the three-dimensional nature of scoliosis. This often reduces their effectiveness and can lead to poor patient compliance due to discomfort.

Modern corrective braces, such as the ScoliBrace, use advanced 3D technology and are fully customized to the patient’s unique body and curvature. Unlike traditional braces, these braces push and guide the spine into better alignment, rather than simply holding it in place.

Bracing is usually just one part of a comprehensive treatment plan. Combining it with therapy, rehabilitation, and scoliosis-specific exercises often yields the best results, as treating scoliosis from multiple angles increases the likelihood of success.
