Delusional disorder is a psychiatric condition characterized by the presence of one or more fixed false beliefs (delusions) that persist for at least one month, without significant impairment in cognition or functioning, apart from the delusions themselves.

Diagnostic Criteria (DSM-5)

According to the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), delusional disorder is diagnosed when:
1. Presence of one or more delusions lasting at least one month.
2. No prominent hallucinations, except those related to the delusion (e.g., a person with a delusion of infestation might have a tactile hallucination of bugs crawling on their skin).
3. Functioning is not significantly impaired, and behavior is not obviously bizarre (unlike schizophrenia).
4. Schizophrenia criteria are not met (no prominent hallucinations, disorganized speech, or severe negative symptoms).
5. Not due to substance use or a medical condition (e.g., delirium, neurodegenerative diseases).

Types of Delusions

Delusions can be bizarre (impossible, e.g., believing aliens implanted a microchip in their brain) or non-bizarre (plausible but false, e.g., believing a spouse is unfaithful without evidence).

Subtypes of Delusional Disorder
1. Persecutory Type – Belief of being targeted, spied on, or plotted against.
2. Somatic Type – False belief of having a physical illness (e.g., rabies circulating in the body).
3. Grandiose Type – Belief of having exceptional abilities, wealth, or importance.
4. Erotomanic Type – Belief that someone (often of higher status) is in love with them.
5. Jealous Type – Unfounded belief that a partner is unfaithful.
6. Mixed Type – Features of more than one type.

Common Symptoms
• Persistent, unshakeable delusions (fixed belief despite contradictory evidence).
• Minimal impairment in daily functioning, except in aspects related to the delusion.
• Lack of insight – The person does not recognize that their belief is irrational.
• Mood disturbances – Anxiety, irritability, or mild depression.
• Social withdrawal – Avoiding interactions due to the delusion.
• Possible but rare hallucinations related to the delusion.
