Sporadic lesions on the hands could be multiple. The reasons could be many. It comes and goes. Goes in the favour of hives or urticarial. It also goes in favour of an insect bite, either a mosquito, bed bug or similar things. With itching it could generally be a bite or an infection. But find out the cause and the pattern, if it come in the morning and subsides in the evening, or if it happens after a bath, or exposure to cold weather, it could be hives caused due to that. First is find out what you did before it happened. Second is consult a dermatologist when the lesion occurs for them to find out the cause. In skin is when there is swelling, there is pain or itching or both. Pain is mainly in infection. Itching is in favour of allergies or insect bites. So find out the problem and accordingly get treated.
