An organized collection of macrophages formed during inflammation found in many diseases. They form when the immune system attempts to wall off substances it perceives as foreign but is unable to eliminate. Such substances include infectious organisms (e.g. bacteria, fungi), foreign objects, keratin, and suture fragments etc.

It usually resolves through organization with scar formation. But caseous masses never resolve, but become calcified and surrounded with connective tissue (incapsulation).
