Severe crowding in adults is often associated with ectopic maxillary canines. This condition affects aesthetics and can negatively impact a patient’s emotional well-being and overall quality of life. In most cases, extractions are required to correct the malocclusion. Typically, the teeth removed are those in the worst condition, often those that have undergone endodontic treatment or are decayed.
In this particular case, molars with crowns were in poor condition and needed to be removed to allow for the alignment of the other teeth. Despite the patient being 30 years old, a lower molar managed to erupt after space was created for it. In March 2015, the maxillary right first premolar and a compromised left molar were removed. Braces were applied, with the exception of the front teeth, to maintain a more aesthetic appearance during the prolonged space closure that was anticipated. This approach proved particularly useful in adults, as it improved aesthetics during the initial phase of treatment.
By January 2016, some of the spaces from the extracted teeth had closed. By November of that year, all spaces were fully closed, and braces were applied to the incisors. In September 2018, the braces were removed, and over the following five years, all old metal fillings were replaced, giving the dentition a more natural appearance. The alignment achieved was stable, with no reopening of extraction spaces during the retention phase.
In November 2016 all braces were bonded. Lower incisors were intruded to correct the deep bite and braces were removed. The space closure of the extraction sites was successfully completed, though despite all efforts, the lower midline remained shifted by 1 mm.
In the mandible, a compromised right molar was removed to make space for the eruption of a wisdom tooth, and braces were bonded. By November, the medialization of the second molar had begun, and by February 2018, the wisdom tooth had erupted, and all braces were in place. In September 2018, the braces were removed, and a fixed retainer was placed.
The wisdom tooth's position and its eruption after space was created for it can be clearly seen.
