The key difference between openEHR and FHIR in my opinion is that where openEHR defines a maximal dataset (archetypes) and asks implementors to constrain them (templates), FHIR defines a minimal dataset (FHIR International release STU3/R4/R5 using the 80/20 rule) and asks implementors to extend (FHIR profiles).

This key difference has trade-offs for both sides. This might have contributed to the adoption of these standards in different communities - openEHR as primarily a storage standard, and FHIR as primarily a transmission and data-in-motion standard.
