Unlike a regular clock, where the hour hand rotates continuously, here it (the blue hand) rotates intermittently. In 1 revolution of the red minute hand, the blue hour hand turns 30 degrees and pauses. The 30-degree rotation of the blue hour hand corresponds 6-degree rotation of the red minute hand. The hour hand rotates when the minute hand points to 12.

This video shows the mechanism that allows that.
Input: red gear shaft carrying red minute hand.
The rotation is transmitted to blue pin wheel carrying blue hour hand through a gear system of 4 gears (red, beige, orange), orange wheel with two black pins, first yellow small gear, green 24-pin wheel with two black pins, second yellow small gear, blue 24-pin wheel.
In 1 revolution of the red minute hand, the orange wheel makes 12 revolutions.
In 1 revolution of the orange wheel, the green wheel makes 1/12 of a revolution.
In 1 revolution of the green wheel, the blue wheel makes 1/12 of a revolution.

When the black pins are not in contact with the first yellow small gear, the outer diameter of the orange wheel keeps the first yellow small gear immobile and the first yellow small gear keeps the green wheel immobile.
When the black pins are not in contact with the second yellow small gear, the outer diameter of the green wheel keeps the second yellow small gear immobile and the second yellow small gear keeps the blue wheel immobile.
