This video is to bring you a closer look at the ASI 0402-WZ Recessed Toilet Tissue Holder for Wet Wall Installation.

Or you can get this in the wall and kind of floating there or temporarily held, then it will go together as so (see video at 2:15).
In between the flanges are several holes that are drilled.
It is through these two hoes in the back that you would run these long 832 flat head bolts.
When you get it installed and you run the screws through the holes, it'll will go into the tapped holes here and draw the clamp towards the inside face of the wall.
Or you could always just take the screws without the anchors and put them through the two dimpled holes and mount it that way.
Units like these you'll see in motels and residential applications.

Make sure that you follow ADA compliance guidelines in terms of where to install these, only if it is going to be in public space (motel, ect.).
