ELIGIBILITY DETAILS:
Nursing Assistant: Candidate must clear GNM Diploma.

Pharmacist: Candidate have clear B.Pharma or 12th pass with Science Stream.

Lab Technician Candidate have a degree of BSc (Medical Lab Technology).

Mahila Parichaayak: Candidate should be literate.

Safaiwala: Candidate should be literate.

DOCUMENTS:
Copies of the following certificates should be sent along with the application. Original certificates should not be sent with the application.

Educational and professional qualification certificates.
Birth certificate (Educational Certificate).
Experience Certificate. if any.
Caste Certificate. If you belong to reserved category.
Residence and Character Certificate.
Copy of Aadhar Card.
Two passport size photographs.
Medical Fitness Certificate (Issued by SEMO/ CMO)
Attach other necessary documents with the application form.

Fill all the columns of the application yourself, In plain and Capital letters.
