Once you gain a mutation you need to relog in order to get another.

Emmet Mountain Disposal Site is my preferred mutation farming location since it has a decontamination arch, but the top of Pylon V-13 in the Cranberry Bog is also a great spot since standing in the ring gives +125 rads and you can jump off the side for easy suicide. Federal Disposal Field HZ-21 is another decent alternative.

You need to be careful not to die too soon after receiving a mutation because there is a bug that may cause you to receive the mutation but it won't be listed, you won't get the effects, and you won't be able to get it again until it is cleared. If you get very low on health and haven't gotten a mutation either die by some other means or apply Starched Genes and clear rads. I like to wait at least 5 seconds after receiving the mutation notification before dying so that it doesn't bug out.

For clearing unwanted mutations Radaway will clear one random mutation at a time and decontamination arch will clear two.
