The discovery was both surprising and humbling.

From experiences like this all we can do is laugh and learn how to be more efficient the next time around. Now, can we explain all the symptoms based on the discovered problem and repair?

Thanks to all who commented on Part 1! All the guesses to the potential problem were valid. Maybe I will do a quick follow-up in part 3 to discuss how to narrow down the issue based on our observations. Of course hindsight is always 20/20, but this is the best way to learn for the next case study that will come along.
