In this video, I’ll show you how to create a student survey using Google Forms. This is a great activity for back to school time.

First, login to your Google account and go to your Drive. Then click New, and select Forms.

You could start from a template, and there are a lot of good templates there, but I'm going to show you how to create a Form from scratch.

The colors and font you see here are the default setting, but those are all customizable and we'll change them later.

First, give your survey a name. I’ll call mine Dr. Z’s Student Survey. It’s automatically saved in your Drive, and as with all Google products, it autosaves so there is no Save button to click.

Under the title, I can put in a description. This is where I typically add my directions or a quick welcome to my survey message.

The first question should require students’ email addresses if they have them. Type in email and Google auto populates that field.

To add another question, click the plus sign to the right. I like to ask students what name they go by in the next question in case they have a preferred nickname or perhaps go by their middle name.

Then I’ll add a place for their last name. I’ll show you why I think that is important later on in the tutorial.

Now let’s look at question types. For my next question, I want students to identify their favorite subject. I could let them type it in, but I’ll offer multiple choice instead. I work with a lot of elementary school teachers who say kids don’t have strong typing skills yet, so I’d rather save my response questions for more important information.

For example, I could ask What responsibilities do you have at home? I like to ask this question because it leads to discussions about classroom responsibilities and class jobs. This is a good open-ended question, so I allow for longer responses by choosing paragraph for the type.

A different style of question might be Which of the following types of literature do you like to read? I would want students to be able to select as many as they want, so I would make this Checkboxes.

Some other neat features to include in my survey are videos and images. I could show add those in just for fun or because they’re nice design elements, or I could make them part of the survey. For example, I could show a video of the four different seasons and then ask students which is their favorite.

Now, let’s say I’m done with the content of my survey, but I want to customize the colors and font. To do so, click the paint palette at the top of the page. You can select a header image for the top of the survey, change the colors, and select a new font. Then preview it by clicking on the eye icon.

When you’re ready to send it out, click the gear icon to set your settings, if needed. Then click Send and choose how you’d like to distribute the survey.

I’m going to take the survey quickly so I can show you how to retrieve students’ response.

Now I’ll go to Responses on the survey and setup a spreadsheet. Anytime someone takes the survey, their responses will automatically go into this spreadsheet.

Earlier I added a question asking for students’ last names. I did that so that I could later alphabetize my students’ responses in this spreadsheet. I click on the Last Name column, then click the A to Z icon, and voila! Now I can easily see my students’ responses in ABC order.
