✅ Overview
Patellar tendon-lateral femoral condyle friction syndrome, also known as Hoffa fat pad impingement syndrome, is a common cause of anterior knee pain, especially in active individuals.
It results from patellar maltracking or an imbalance between the medial and lateral vastus muscles, leading to impingement of the superolateral aspect of the Hoffa fat pad between the inferior patella and the lateral femoral condyle.

✅ Clinical Presentation
Anterior knee pain worsens with hyperextension
Point tenderness at the inferior pole of the patella
Thickened fat pad, making patella palpation difficult
Associated conditions: patella alta and lateral patellar subluxation

✅ MRI Findings
Focal high T2 signal (edema) at the inferolateral patellofemoral joint
Involvement of the superolateral portion of the infrapatellar fat pad
Possible cystic lesion between the lateral femoral condyle and lateral retinaculum
Strong association with lateral patellar subluxation and patella alta (seen in ~90% of cases)
