I had to do some experimentation before eventually figuring out how I should ghost-step to hit Goods more consistently, although some luck is still involved with Bads due to variance from human error.

In order to keep getting Goods, you want to hit the arrows earlier by exactly one 1/24 beat (1/6 in osu terms). For Bads, you hit just VERY SLIGHTLY earlier, although those will naturally crop up thanks to the variance mentioned above. As for the Misses, it's safer to get from the very last arrows, but how many you let go depends on the number of Bads you've gotten throughout the song. Due to the (historic) nature of PIU's judgment window, you want your steps to be right on beat or earlier; if you start your ghost-stepping just a tiny bit later, you're much more likely to get Great or higher, which immediately throws your run out of the window.

My assumption is that you cannot exceed a total of 9 notes as Bads and Misses combined before your life bar is fully drained, and the closest you can get below the 180k score threshold is 6 Bads and 3 Misses, as shown in this video. Of course, if Misses indeed hurt more than Bads, this assumption falls apart.
