In this lecture we talk about the approach to patients in the emergency department (ED). Often times patients in the ED may have multiple chief complaints or may be a poor historian. As a result this can make taking a concise history very difficult. This is a problem given that the goal of an ED encounter is to be in and out of the room in 20 minutes with a clear idea of what labs, imaging, and treatment to initiate. We recommend that after the chief complaint a critical differential diagnosis should be developed. That way the provider can focus his/her history and physical on the life threats for each chief complaint. It is important to be weary of wild cards and to make sure to constantly reassess your differential in order to prevent early anchoring.
