Scoliosis bracing is an established treatment for scoliosis, with a division between "believers" and "non-believers" due to varying brace designs and outcomes. Many people assume that all braces are the same if they are made of plastic and worn around the torso, but that’s not the case. It’s similar to how we have different types of cars—race cars, trucks, and luxury cars—all designed for specific purposes. This analogy applies to scoliosis braces as well.
Braces are especially beneficial for spines that are still growing, but properly designed braces can also help patients of all ages, including adults and those with late-stage progression. The modern approach to bracing offers a broader spectrum of treatment options.
One key factor in scoliosis bracing is the "dose of time"—how long a brace is worn. The duration depends on the specific brace, the patient, and the treatment goals. Some braces are worn full-time, while others are designed for nighttime use.
The type of brace prescribed often reflects the treatment approach chosen. For instance, the Boston Brace is typically used for moderate scoliosis in patients aged 12 to 18 with curves between 25 and 40 degrees. It is usually prescribed in more traditional treatment approaches. However, modern non-surgical treatments employ corrective braces that aim to reduce the size of the curve rather than just slow its progression. One example is the Scoli Brace, but there are other types depending on what the doctor deems most effective.
Traditional braces like the Boston Brace are rigid and use a three-point pressure system to straighten the spine. While effective at slowing curve progression, they have limitations, including discomfort, muscle weakness, and pain. As the curve worsens, the brace may become harder to wear, signaling that the treatment isn’t addressing the condition properly. Even the "3D" version of the Boston Brace doesn’t address the three-dimensional nature of scoliosis.
In contrast, modern corrective braces like the Scoli Brace are custom-designed for each patient. They address the three-dimensional aspects of scoliosis and use biomechanical correction to improve the spine's position. These braces can be adjusted as the patient’s condition changes, providing a more personalized and effective treatment option.
Corrective braces are part of a broader conservative treatment plan that combines bracing with therapy, rehabilitation, and exercises. This coordinated approach aims to achieve the best possible results for the patient.
Another type of brace is the nighttime-specific brace, such as the Charleston or Providence braces. These braces are designed for use only during sleep, as they place the patient in a bent position. While they can be effective for some patients with smaller curves, their use is limited since they can only be worn while sleeping and may not provide a "stronger dose" of treatment.
Finally, soft braces like the SpineCore brace are used for small, flexible curves to slow progression. While more comfortable than rigid braces, soft braces do not have the ability to change the shape of the rib cage or improve spinal alignment in the same way as corrective braces.
Though soft braces can help, they are limited in their ability to correct scoliosis. A corrective brace, even at a smaller dose, is typically more effective. No brace alone can correct scoliosis, but when used in conjunction with a comprehensive treatment plan, bracing can help reduce the curve size and address scoliosis on a structural level.
