solitary neurofibroma is a benign peripheral nerve sheath tumor that arises sporadically and is not associated with neurofibromatosis type 1 (NF1) in most cases. It’s one of the most common types of neurofibroma and typically presents as a single, slow-growing lesion 🧠.

🧬 Key Features
Origin: Composed of Schwann cells, fibroblasts, perineurial cells, and mast cells embedded in a collagenous or myxoid stroma

Histology:

Unencapsulated, low-cellularity spindle cell tumor

Often shows a “shredded carrot” appearance due to interwoven collagen bundles

Positive for S-100 protein (Schwann cell marker) and CD34 (stromal marker)

Location: Commonly found on the skin of the head, neck, trunk, or proximal limbs

Size: Typically 2–20 mm in diameter

Texture: Soft, rubbery, or flaccid papule or nodule

Color: Skin-colored to pinkish-brown

🩺 Clinical Presentation
Usually asymptomatic

May cause cosmetic concern or local discomfort

Characteristic “buttonhole invagination”: When pressed, the lesion dimples inward—a diagnostic clue

🧠 Differentiation from NF1
Solitary neurofibromas are sporadic and not inherited

Unlike NF1, they do not present with multiple lesions, café-au-lait spots, or other systemic features3

🛠️ Management
Observation: Often sufficient if asymptomatic

Surgical excision: Considered for diagnostic confirmation, discomfort, or cosmetic reasons
