I would like to present to you a new surgical access that we published this year. Over the decades we have been polishing our technique to make surgery less invasive and better for our patients. That's where the Modified Subcrestal Buccal Flap Approach to All-on-Four (Allon4) was born. We know that the gold standard of minimally invasive surgery is flapless guided surgery, but we also know that sometimes it is not possible to do it this way, either due to the reduced mouth opening, large inclinations of extra-long implants, some situations that lack good gingival quality, in short, open surgery is still necessary in many cases. But if we look carefully, the way we make the incision and mucoperiostial detachment for allon4 is identical to bone graft surgery - with large detachments. We discuss in this article the possibility of a palatine-only exposure, killing the entire incersion of the muscles and mucosa in the vestibular region of the alveolar edge, decreasing the surgical time and postoperative edema. IN addition, the entire line of incision and suture is by vestibular, bringing benefits for healing and prosthetic management.
