You'll need:
-PS3 Console (duh!) (You'll also need a controller & its accompanying USB cable)
-SSD (or replacement HDD)
-USB with the firmware (FAT32 formatted)
-Screwdriver

How to perform this operation:

Replace the HDD:
1. Fully power down the PS3, unplug EVERYTHING, then flip it to its underside
2. Remove the plastic flap on its underside (should be obvious on where it is), then unscrew the single screw holding it in place.
3. Remove the small piece of the front faceplate holding the HDD caddy in place.
4. Undo the screws from the caddy (Holding the HDD in place), then remove the old drive.
5. Insert the new drive, and re-screw the screws in the caddy. You WILL need to line up the HDD/SSD with their respective holes in the underside, so lining it up properly is VERY crucial.
6. Re-apply the HDD caddy in place, then put the front faceplate back on.
7. Upon doing so, re-apply the screw over the flap, and put said flap back into place.

Install the firmware:
1. Follow the link posted here in the description.
2. If it's not, you need to save the file as PS3UPDAT.PUP
3. On your FAT32 formatted USB, create a new folder named PS3 (This has to be in all caps).
4. In said folder, make a new folder named UPDATE (Yes it has to be in all caps).
5. Put the firmware in the UPDATE folder.
6. Transfer the USB to your PS3, and power on the console.
7. Follow the on-screen instructions to install the firmware/OS on your PS3/
8. When all is said & done... you should have your PS3 up & running again!!
