Been Waiting a Long Time for This, one of the most challenging Lich King achievements in Icecrown Citadel and was the last one I needed for my Glory of the Icecrown Raider. Once I figured out a good place to stand, it was just a matter of waiting for the plague to stack.

This can't be done solo unless you can summon an additional target like a pet. Necrotic Plague will not be cast by him if you're the only target. If solo, you need a pet or minion to attack the Lich King. Soon thereafter, he should casting Necrotic. In my attempt, I had a party mate who got knocked off the edge and died.
