How to Apply for Combined Plus Two Original /Revaluation /SAY/Improvement Certificates

List of documents required for submitting the application form The documents given below should be submitted by the applicant.
1. The prescribed application form attested by the principal of the institution where the student had registered for Higher Secondary second year exam.
2. Original mark sheet of + 2 exam and that of improvement exam.
3. Original challan receipt of Rs 40 paid at government treasury.
4. Self addressed and stamped envelope for speed post or register post for the certificate to be sent.
