Context: Muzan and his brother have a special bond, they both love and tease each other gleefully just like most brother relationship would do, it's his brother's habit to pat Muzan's head whenever he achieve something good.
Muzan can be a sensitive person sometimes and only for specific reason, he can resist on crying for any other reason but can't whenever he had deja vu's about his family, they we're the only one who was there for him during his human life after all.

Koku is one of the main people that remains Muzan about his family specifically his brother and when he talked about his new project/experiment or something that he's proud of doing, Koku will always give a smile of a big brother that could potentially break Muzan into tears.
