Chadi Ayoub, MBBS, PhD, an Imaging Cardiologist and Associate Professor of Medicine at Mayo Clinic in Phoenix, AZ, reviews his article appearing in the October 2025 issue of Mayo Clinic Proceedings. He and the authors report use of an IL-1 inhibitor in a patient with constrictive pericarditis, who was not able to tolerate standard steroid treatment, and who previously required weekly thoracentesis for heart failure due to the constrictive physiology. The patient showed significant symptomatic improvement, normalization of inflammatory markers, and most significantly resolution of pleural effusions such that thoracenteses were no longer required. Following discontinuation of the treatment by the patient, her symptoms returned within 4 months which prompted reinstatement of the IL-1 inhibitor that demonstrated repeat improvement. Dr. Ayoub notes this treatment shows great promise for treatment of patients with constrictive pericarditis, but notes early initiation of the treatment is paramount and that further study is needed to determine best practices for the use of IL-1 inhibitors in this patient population and better understand appropriate patient selection.
