A RIFA (or very similar!) capacitor failed in a piece of old vintage equipment I had just bought. I plugged it in and an acrid cloud of the magic smoke was released!
This was found to be a capacitor within an in-line filter. The model of the filter is a CDE HPF 300CET made in the USA. Probably early 1980s.
