Care coordinators are available at NorthShore Health Centers to help you understand your health, answer questions, and coordinate with other health providers to get you on track to managing your health. They can help educate you about chronic conditions like diabetes, high blood pressure, and cholesterol. If you need to see a specialist or need to receive treatment outside of NorthShore, they are available to help coordinate getting you scheduled at that facility. If other barriers are impacting your health, they will direct you to resources to help alleviate those barriers so you can manage your health.
