As a child care provider, obtaining proper consent forms from parents is crucial to ensuring the safety and well-being of the children in your care, and protecting the organization from litigation.

Crafts makes managing consent forms a breeze. With Crafts you can create consent forms, that become mandatory for parents to complete during registration. The benefit of this for the daycare program is that all consents are easily accessible through CRAFTS when you either go to the Consent Status or Enrollment Form reports on the Report menu.

To create consent forms, you can either use step 13 of the set-up wizard or go to the ‘Admin Menu’ and select ‘Consent Forms’ under the Registration section. Clicking Consent Forms will list exiting consent forms in the system, allowing you create new forms or view and edit any forms in the system.

When creating or editing consent forms, the code field is mandatory but not visible to parents and is only used by staff as a quick identifier of the consent form. Next, select whether the form is active and enter the order it will appear in during registration. When you’re finished, hit ‘save consent form’.
