After the events in Portugal, besides a 50k $ fine and the disqualification from Portugal's race, Nigel Mansell received a race ban that didn't allow him to compete at the Spanish Grand Prix.
McLaren and Senna were pretty upset about Mansell, saying that he crashed Senna on purpose but they would face backlash on Friday.
During the Friday Qualifying session, Gregor Foitek crashed his Rial hard against the barriers and the session was stopped. When Foitek crashed, Senna was on his warm-up lap but flags weren't displayed yet.
However, the black flags (red flags nowadays) were shown during Senna's qualifying lap, and he ignored them, driving flat out at the crash scene. He would only stop after crossing the red flag at the start/finish line, right after completing his lap.
In total, Senna ignored 8 black flags during the course of the lap, with protests from Ferrari since Mansell was banned for ignoring a single black flag three times (therefore 3 black flags).
The punishment for Senna was a 20k $ fine and the deletion of his qualifying times up to that point. Since there was still another qualifying session on Saturday, Senna achieved his 40th pole position that day.
