Hormonal contraceptives

Uses- Prevent pregnancy

Combined oral contraceptive - inhibit ovulation
indicated - in newly married women

types:
monophasic pill
biphasic pill
triphasic pil

Progestin only pill - increases cervical mucosal thickness and make hostile to sperm penetration
indicated - in women having contraindication for estrogen, smoker, more than 35 years of age, risk of thromboembolism, sickle cell disease, lactation, seizure

Emergency pill - Inhibit implantation
indicated - unprotected sexual intercourse including rape, failed condom, failed contraception.

Implants:
intramuscular implants - eg DMPA
subcutaneous implant - Norplant for 5years

Adverse effects
Nausea
vomiting
edema
mastalgia
worsens migraine
breakthrough bleeding
withdrawal bleeding failure
weight gain - more with progestin with androgenic activity
acne - more with progestin with androgenic activity
hirsutism - more with progestin with androgenic activity
risk of thromboembolism - because of estrogen increases clotting factors
risk of MI - because of estrogen increases clotting factors
risk of stroke - because of estrogen increases clotting factors
cholestatic jaundice
gall bladder stones
hepatic adenomas
risk of breast and cervical cancer
reduced risk of endometrial and ovarian cancer
