Magnitude and location of sagittal plane curvatures and alignment differ from one person to another which is regulated mainly by the pelvic shape. In case of deviation from the ideal, compensatory mechanism are activated in an attempt to position the head over the pelvis and to maintain a forward gaze. Sagittal malalignment and accompanying compensations may be the underlying cause of back pain for many patients with degenerative conditions, and may drive mechanical complications after spinal fusion. Thus, treatment success and avoidance of complications rely on the subject-specific evaluation and adapting a personalized treatment planning.

Planning of spine surgery has received increasing attention in the past decades and optimization of it with new digital tools is being developed to achieve a better spinopelvic alignment and outcome in the future.
