The Interdisciplinary Home-bAsed Reablement Program (I-HARP) is a unique aged care service model that aims to help older people with dementia remain healthy and independent in their own homes.

I-HARP is funded by the Australian government and is currently being trialled by the University of Sydney.

Featured are Mrs Walentyna Plateris and her daughter Anita who participated in I-HARP between 2016-17.

Sadly, Walentyna passed away on 6 June 2023, after her 95th birthday earlier in the year. Anita said “Mama felt very proud of her involvement in your project. Her beaming smile at the end of the video speaks volumes.”

The I-HARP team is grateful for Walentyna and Anita’s contribution to helping others better understand about the importance of and what can be achieved through rehabilitation and reablement in dementia care.
