The SI Joint is weird-- it has cartilage but is not supposed to move like a normal joint, and in early stages it can appear normal while laying flat for imaging and yet have severe destabilization and pain. Sometimes it can appear along with other injuries to the spinal discs, hip joints, or cluneal nerves, which are all within an inch of each other, so it is sometimes difficult to discern these different pathologies and sometimes they can all be affected to some degree (as when a bridge collapses, multiple structures destabilize). SI joint injections with regenerative agents can provide therapeutic relief within the joint and stabilization of the surrounding ligamentous capsule, as well as diagnostic confirmation and information on how much the SI joint injection generates and replicates the pain.
