How to find/get to/travel to:
Whitaker Forest Dungeon, Caves of Unrest Dungeon (and how to open/unlock Caves of Unrest,) Lava Run Dungeon, Gills Grotto, and Roaring Hollow Dungeon.

Whitaker Forest is located in Greenguard, Hertwood Forest.
Caves of Unrest is located in Doomwood, Doomwood Forest.
Lava Run is located in Ashfall, Mt. Ashfall Camp.
Gills Grotto is located in Mysterious Islands, Lolosia, Capt. Rhubarb's Pirate Excursions, Mysterious Caves.
Roaring Hollow is located in Dragon Graveyard, Zakaru's Dragon Graveyard.

The Fallen Destiny Title is a Reward (an automatic Title) for finishing Guardian Alister's Fallen Guardian Quests.

Journal Pages are Loot Items dropped by the Boss in each Dungeon. You will have to go through each Dungeon several times to collect 30 Journal Pages from each Dungeon.

The Fallen Guardian's Trunk Craft Shop is located in Zakaru's Dragon Graveyard. (Take a right by Zakaru and head to the Defiler Bandits, to find the unmarked chest.)
You buy each item for $0 Gold.
