Scoliosis causes an unnatural spinal curvature that disrupts the alignment of the spine, surrounding nerves, and muscles. This imbalance doesn’t just affect posture—it alters how the muscles function. Over time, this can lead to noticeable strength differences between each side of the body.

A common question patients ask is: which side is weaker—the concave or the convex side?

It was once believed that the concave side (the inner curve) had stronger muscles pulling the spine inward, while the convex side (outer curve) was weaker and overstretched. This led to treatment approaches focused on strengthening the convex side and lengthening the concave side.

However, modern research shows this is not entirely accurate. In typical idiopathic scoliosis, the curvature isn't caused by muscle imbalance. Instead, the muscles are responding to changes in spinal structure. Muscle imbalance becomes more pronounced as the curve progresses, but it isn’t the root cause.

In a recent study using MRI to assess muscle strength, researchers found that the concave side muscles were actually weaker, even though they were shorter and tighter. Meanwhile, the convex side muscles were stronger, even though they were stretched. This proves that tight doesn't always mean strong, and stretched doesn't always mean weak.

As scoliosis progresses, muscle imbalance can worsen. That’s why early treatment is essential. Treating scoliosis involves more than just exercise. It may include chiropractic adjustments, scoliosis-specific exercises, bracing, and physical therapy.

These conservative treatments aim to reduce the curve while improving spinal function. A straighter spine gives muscles better leverage, which improves strength and balance.

The goal isn’t perfect symmetry—because scoliosis is a progressive condition with no cure—but early and proactive treatment can limit curve progression and reduce muscle imbalances.
