This was a challenging case due to extreme aortic tortuosity, hindering coronary artery engagement. Using a 45 cm long femoral sheath and the "armored guide catheter technique" (guide manipulation while a 0.035 wire is inserted in the catheter lumen) the right coronary artery was engaged. A distal RCA true bifurcation lesion was found using intravascular ultrasound and was successfully treated using the DK crush technique.
