Look out for these hidden ethical issues in PLAB 2. A young person below the legal age of consent of 16 years presenting with swellings in the groin should of course prompt you to consider a sexually transmitted infection by exploring their sexual activity. But do not forget to ALSO consider the possibility of abuse by asking a few questions about their partner(s).

Sexual abuse can only be excluded once we determine that it is consensual sexual activity with a peer of a similar age group (16, 17, 18, etc is fine) and with no clear difference in power.

Much larger age gaps and/or a clear difference in power or mental capacity between a young person and their sexual partner require further investigation - in particular when the relationship is incestuous, or is with a person in a position of trust (such as a teacher, sports coach, or minister of religion), or there is a concern that they are being exploited. In such cases, you should follow appropriate child protection procedures and refer to a paediatrician if necessary.
