Geriatric Care: Home Based Primary Care

The Home Based Primary Care program is designed for Veterans who have complex medical needs and are unable to travel to a medical center. Multidisciplinary care teams work together to provide primary care to the Veteran in their home. Participants in the care team will visit the Veteran’s home within a 35-40 mile radius of the medical center as needed and educate the caregiver about the needs of the Veteran. The goal of this VA program is to help Veterans remain independent in their homes near their family as long as possible. Since Home Based Primary Care is part of the VHA Standard Medical Benefits Package, all enrolled Veterans are eligible so long as they meet the clinical need for the service and it is available.
