Have your Coop buddy run to the end of the level shown in the video - you will go to the beginning of the level by the stairs. As soon as your Coop buddy hits the checkpoint by the Missile it will erase the previous level leaving you an opening to the current one you are on and will allow you to fall out of it and onto the rooftop. Use your triggers (aim) and move the thumb sticks the way you want to steer.

Note 1: In order to survive falling through this map, you must be online playing Coop - there are places like Pakistani Embassy, Voron Station, Opium Farm, and Hawkins Seafort which do not require falling to look around and can be done solo.

Note 2: The other place you can fall out of the map into the 2nd, 3rd, and 4th stage is after you shut down the power on the 3rd stage you will walk through a mine field - go to the Wall Jump but DO NOT activate it or you will not be able to go back through the mine field.

When you get up to the Wall, it loads stage 4 and erases most of the the objects on Stage 2. You can go back through the mine field and go the way you came into the 3rd stage - turn on your night vision, jump over the railing and fall into whichever stage you want based on your aiming (triggers/moving the thumb sticks).

Important - make sure you do not die after opening up the gate to the mine field or your checkpoint will lock the gate for good which will require restarting the mission.
