I wanted to find a way to complete the game with Melina still alive and I found a way. Just accept the flame of frenzy before you go to the flame of giants and youll be able to burn down the door to the throne yourself without Melina having to sacrifice herself. After that just use Miquellas needle in Farum Azula near the dragonlord boss to get rid of the flame of frenzy. Then I just collected the rune of perfect order from goldmask and got this ending.
