The following video shows how to obtain all One Third of Umbilical Cords in a game cycle of Bloodborne.
First of all, after defeating Gascogine you have to meet Arianna and send her to a safe place.
You will find Arianna in the Cathedral Ward. It is better sending her to Oedon Chapel to obtain an Umbilical Cord in a safer way.
The first One Third of Umbilical Cord is placed in the hidden Old Workshop. This area is accessible after defeating the boss Blood-Starved Beast.
The second One Third of Umbilical Cord could be obtained from Iosefka. You have to kill Iosefka at Iosefka's Clinic. However, this should be done after having defeated Rom the Vacuous Spider at the Moonside Lake (Byrgenwerth). You can access to Iosefka's Clinic from the Forbidden Woods.
If you sent Arianna to Iosefka's Clinic theoretically you should obtain another One Third of Umbilical Cord here by killing fer (after having killed Rom).
The third One Third of Umbilical Cord is obtained from Arianna. If you sent her to Oedon Chapel you will find her on your way back to the arena of Father Gascoigne. She has had a baby. Kill the baby to obtain the Umbilical Cord. This will happen right after killing Micolash at the Nightmare of Mensis.
The fourth and final One Third of Umbilical Cord is obtained after killing Mergo at the Nightmare of Mensis.
