Thinking you might have ADHD and seeking diagnosis as an adult can be a really positive but challenging turning point. The process is a rollercoaster, with lots of twists and turns to navigate as you go through the process of doctors, assessments, and forms. It can feel hugely overwhelming when you're already feeling overwhelmed!

This video is the second of two parts about the pre-diagnosis questionnaire you need to fill out in detail once you've been to see your GP. These forms cover the major symptoms and criteria for ADHD, and you'll need to evidence and explain that you suffer from at least 5 criteria in each category before you can be considered to have the condition. These will need to be completed before you have your formal assessment with a psychiatrist. In this second part I cover 'hyperactivity' and 'impulsivity' (the last video was about inattention) and give examples of my answers from my own documentation to help you think about what you might write on yours.
