How to place a 12 lead ECG and its electrodes. Limb leads and precordial chest leads explained.

A 12-lead ECG (electrocardiogram) records the heart's electrical activity from 12 different angles using 10 electrodes placed on the body. Here's the standard placement:

Limb Leads (4 Electrodes)

1. RA (Right Arm) – On the right wrist or just above the right wrist on the forearm.


2. LA (Left Arm) – On the left wrist or just above the left wrist on the forearm.


3. RL (Right Leg - Ground lead) – On the right ankle or lower right leg.


4. LL (Left Leg) – On the left ankle or lower left leg.



These four electrodes form the frontal plane leads:

Lead I: LA(+) – RA(-)

Lead II: LL(+) – RA(-)

Lead III: LL(+) – LA(-)

aVR (Augmented Vector Right): RA(+)

aVL (Augmented Vector Left): LA(+)

aVF (Augmented Vector Foot): LL(+)


Precordial (Chest) Leads (6 Electrodes)

These leads capture heart activity in the horizontal plane:

1. V1 – 4th intercostal space, right of the sternum.


2. V2 – 4th intercostal space, left of the sternum.


3. V3 – Midway between V2 and V4.


4. V4 – 5th intercostal space, midclavicular line (below the nipple in men).


5. V5 – Same level as V4, at the anterior axillary line.


6. V6 – Same level as V4 and V5, at the midaxillary line.



These precordial leads (V1-V6) give a view of the heart’s electrical activity from different angles across the chest.

Proper placement ensures accurate ECG readings, helping in diagnosing heart conditions like arrhythmias, myocardial infarctions, and conduction abnormalities.
