It depends on how severe the chocolate cysts or ovarian endometrioma is. If it is a minimal ovarian endometrioma and if they are surgically treated by ablating the endometrial tissue from the endometrial bed. Most of the times the conception rate will be high, before it causes a devascularisation and follicle loss if it is treated, the conception rates should be high. But in cases of severe ovarian endometrioma, it causes anatomical distortions, tubal distortions and because of the increased interleukins and progesterone in the follicle fluid, there will be a poor oocyte quality or embryo quality and there will be embryo development and receptivity because of which the conception rates will be poor. So in severe endometriosis, the conception rate is very poor.
